Role Description
This is a contract role for an Occupational Therapist or Occupational Therapist Assistant (COTA) at Snap Health in Garland, TX & Surrounding Areas (Richardson, Sachse, Rowlett). As part of the team, you will be providing patient care in a Home Health, on-site setting. Your day-to-day tasks will involve delivering quality care and ensuring optimal patient outcomes.
Responsibilities:
· Evaluates the patient’s functional status and occupational therapy needs. Assists the physician in the development of the therapy plan of care.
· Provides services within the scope of practice as defined by the state board of occupational therapy, in accordance with the plan of care, and in coordination with other members of the health care team.
· Treats patients to relieve pain, restore function, and / or maintain maximum functioning using acceptable standards of practice.
· Instructs and informs participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
· Visits patients according to Plan of Care, completes progress notes timely.
· Provides supervision of COTAs/OTAs
· Participates in discharge planning for patient.

Qualifications:
- Must be currently licensed to practice as an Occupational Therapist or COTA in the STATE OF TEXAS
- Must possess and maintain valid CPR certification.
- Valid driver’s license, auto insurance and reliable transportation
- Experience in a home healthcare setting is a plus
- Ability to work independently and as part of a team.
- Proficient in using mobile apps for organizational purposes.
