Job Summary:
Snap Health is seeking a dedicated Physical Therapist Assistant (PTA) for a full-time home health position. The PTA will provide skilled physical therapy services to patients in their homes under the supervision of a licensed Physical Therapist, helping patients improve mobility, strength, and functional independence.
Responsibilities:

• Provide physical therapy treatments in accordance with the established plan of care.
• Carry out therapeutic exercises, gait training, balance activities, and functional mobility training.
• Monitor patient progress and report changes to the supervising Physical Therapist.
• Educate patients and caregivers on exercises and safety techniques.
• Maintain accurate and timely documentation of visits and patient progress.
• Communicate effectively with the interdisciplinary care team.
• Adhere to agency policies, procedures, and regulatory standards.

Qualifications:

• Current Physical Therapist Assistant license in the State of Texas.
• Minimum 1 year of PTA experience preferred (home health experience a plus).
• Strong clinical, communication, and time-management skills.
• Ability to work independently in a home health setting.
• Valid driver’s license, reliable transportation, and auto insurance.
• Proficient with mobile apps and electronic documentation systems.
