Role Description

This is a contract role for a Physical Therapist or Physical Therapist Assistant (PTA) at Snap Health in Garland, TX and surrounding areas (Richardson, Sachse, Rowlett). As part of the team, you will be providing patient care/home care in an on-site setting. Your day-to-day tasks will involve delivering quality care and ensuring optimal patient outcomes.

• Conduct thorough evaluations of patients’ physical conditions and develop personalized treatment plans.
• Work with home health patients to improve mobility, alleviate pain, and enhance overall well-being.
• Collaborate with interdisciplinary healthcare teams to ensure comprehensive patient care.
• Utilize the Snap Health mobile app to stay organized and manage your caseload efficiently.
• Provide excellent patient education and support to promote optimal recovery.

Qualifications

• Must be currently licensed to practice as a Physical Therapist or PTA in the STATE OF TEXAS
• Must possess and maintain valid CPR certification.
• Valid driver’s license, auto insurance and reliable transportation
• Experience in a home healthcare setting is a plus
• Ability to work independently and as part of a team.
• Proficient in using mobile apps for organizational purposes.
